You can now save an insurance entry on the Admin > Insurances Page even if certain required fields are left blank. This allows you to capture partial information while waiting for complete details. However, invalid addresses will still prevent saving. Incomplete entries will be marked as "Incomplete" in the system.

Following CMS regulations, the updates below will take effect on January 1, 2025. For a more in-depth breakdown of Medicare's Final Rule, please watch the recording of our recent webinar.
CMS Adds New CPT Codes for Direct Caregiver Training
For 2025, CMS has added three new CPT codes for Direct Caregiver Training. These codes can be billed when a caregiver (family member, friend, neighbor, or guardian) is trained to provide care for a patient with a chronic illness or disability.
Updates to Existing CPT Codes
Descriptions for the following CPT codes, already present in the system, have also been updated to align with CMS requirements.
New Modifier for Audio-Only Billing
Starting January 1, 2025, Medicare will no longer accept Modifier 95 for audio-only telehealth. Use Modifier 93 for audio-only sessions. Example: For codes 99441, 99442, or 99443, use Modifier 93 for phone consultations and Modifier 95 for video consultations.
Therapy Threshold Updates
To help you navigate the new year, here’s a quick overview of important Medicare threshold updates.
Therapy Thresholds:
- Physical Therapy & Speech Therapy: $2,410 combined
- Occupational Therapy: $2,410
Medical Review Thresholds:
- Physical Therapy & Speech Therapy: $3,000 combined
- Occupational Therapy: $3,000
Plus, now in SOAP 1.0, you’ll receive alerts when a patient’s spending approaches or exceeds these thresholds. KX modifiers can also be applied directly from these alerts for both “approaching” and “exceeded” thresholds.
CCI Edit Updates
The system has been updated to automatically apply the latest version of the CCI edits appropriately for dates of service on or after Jan 1, 2025.
Medicare Conversion Factor and RVU Calculations
The 2025 Medicare Physician Fee Schedule conversion factor has been set at $32.3465. Additionally, starting January 1, 2025, Medicare will update its Relative Value Unit (RVU) calculations.

This could be due to either:
- The appointment wasn’t checked in
- Clinic location mismatch between the appointment and note
- The appointment was canceled/no-showed, then later edited to be checked in.

You can only delete drafted notes. Finalized notes are legal pieces of medical documentation that cannot be deleted. If a note was finalized in error, you can addend the note and select “No Charge This Visit (SOAP 1.0”) or “Do Not Bill (SOAP 2.0)” and confirm with your biller if you should finalize or finalize and rebill.
